No mods, slightly adjusted economy only but that wouldn't effect this.
Hardware is older... only 3 people on during the issues

I'm guessing it has something to do with the issue that causes blood to remain on the screen as I had that bug shortly after. Also my hands went to the sky trying to melee a zed... had to relog to remove the blood from my vision. Didn't test to see if the mask functioned after the relog as I had left the gas zone.
