To reproduce it, just open the 3DEN Editor and place an "Edit Terrain/Environment Object" module on the two Saigon USA Embassy buildings and set the object to "Destroyed" (or "ruins"?).
It will be able to see that one out of the two Embassy building objects "falls over" as if it were a wall that just had been destroyed.
You can then save the mission, and play it. The building will remain destroyed when you start the mission, and also while you play it.
In theory this issue should also be possible to reproduce by simply dealing enough damage to the affected building until it becomes destroyed.
