This is the second time it has happened. First time I was in a room with no windows, swapping items in my inventory, and suddenly, died. Second time, I was on the 3rd story near a window crouched; took handcuff keys (inventory item pic doesn't show all the time??) into hands, then returned to backpack, died.

Both times I returned to the spot I died. First time was within minutes, and no body, no gear, nothing?? Second time, returned again this time 3 minutes till server reboot, so had to wait for server reboot. Returned to spot of being killed, and this time there is still no body, but some gear remains.

The second time I thought for sure the bug in the handcuff keys (no inventory icon) caused the "server side killing", but is it possible I was actually killed by a player?

It would be a SUPER COOL feature if when you died there was a short kill camera that showed who killed you!! Then I would know for sure it wasn't a glitch. ;)
