Many of us have had this bug where your guns Iron sight is broken or off.
It seems to happen when your ADS and get hit in the arms/hands when you just aimed at someone and he hits you in your arm at the same time.
Maybe it happens while we shoot and get hit almost at the same time.

Also have a similar bug where our aim is way off when we come out of uncon state.
ADS your gun and shoot at your target while he shoots your and hits your arm at similar timing. Easy to produce right? ;-D Good Luck
The only fix is to respawn or relog.
The other bug where you wake up from uncon and pick up your gun, let's say a mosin with a PU get's inaccurate.
I have had it happen quite often and others that I know. This bug might be related to the other bug.
