Steps To Reproduce
- Bind the EpiPen to a quick slot
- While sprinting, take to hands, stop sprinting but remain moving, then inject the pen while continuing to hold down the "move forward" key
- The injection animation will complete, but the effect of the EpiPen will not apply
- Observe that the EpiPen disappeared from inventory
- Take anything from your inventory to your hands and try to use it
- Observe you are unable to use any item you've taken to your hands
- Relog (if you're not being attacked by wolves and bleeding to death)
- Observe the EpiPen is back in your inventory

This damn bug finally caught up with me and got me killed, so I'm reporting it. It's been around since the "new character controller" was introduced.

Yes. You have to be sprinting and time the injection for the instant you've run out of stamina, but don't stop moving forward during the injection animation. This has happened to me so many times.
I got a new PC and started playing on that platform and was able to reproduce it there, so it's not just a PS4 bug.
