This isn't necessarily a bug. It's more of a uneducated game-play mechanic. I've been using a dust mask to avoid picking up sicknesses from other players, but I've noticed that I keep getting sick, so I swapped to a surgical mask just to test it out, and I stopped getting sick. Surgical masks aren't for surgeons' protection, they are for the patients' protection. They prevent spit and sweat from getting inside a patient's open wounds and internal organs. A dust mask is much more protective for the wearer, preventing many airborne particles from being inhaled. It's just a small detail that irked me.

Hello Fundesu.
Both surgical mask and the dust mask have certain percentage of protection against illnesses configured. Currently the dust mask has higher protection than the surgical one. However, they do not completely protect you from the sicknesses and if you are around player characters that are ill (and are audibly coughing), your character will likely end up ill as well.
