You can loose your gear by setting beside you. I found a jacket. I put it on to see if it was better camouflage than the old one. When I whet to transfer my gear it was gone. Jacket and all the gear that was in it.

I whet down stairs in the room just below where I was at. The jacket was stuck to the celling. No way to get to it or get it down.

Also having trouble transfer gear from pack to jacket or vice versa. I would have reload the game to fix this one.
