Can you try to verify the integrity of game files through Steam and clear the contents of your DayZ folder located in the Documents?

Hello PristineKiwi.
Have you checked if this could be caused due to a hardware issue on your mouse? Does the middle click work correctly in other applications? And are you able to check if you can aim down sights with another mouse?

Middle mouse works fine everywhere else, and my scroll wheel definitely is noticed in dayz, but it is only the middle mouse button that is not responsive in dayz.

It also used to work when leftshift was an alternative to middle mouse button aiming down sights; also did a fresh install
