What do you mean by "This is a form to report an issue from the PC VERSION of the game - for Xbox issues use the "New DayZ Xbox Bug Report" form (delete this prior to creating the ticket)"

THIS IS PC VERSION!!! I NEVER STATED THIS IS AN XBOX ISSUE.

Hello leon1721.
The description message is there by default for the pc version form due to xbox reports being filed under the pc version often, it is not aimed towards you specifically but in general.
As for the issue you are experiencing, have you experienced this issue on the previous 0.63 versions as well? Also, have you tried to do a clean install of the latest video drivers available (unless you are using the latest version already) to see if it has any effect on the issue you are experiencing?
Regards,
Geez
