When you slide down the ladder at Kamensk Military Base ( South side ) you fall though the ground and hit water. I've not died when it happens.

To get back on the ground all you have to do it relog.
I think to fix this you need to raise the ladder up a little bit or flatten that slope so the bottom part of the ladder isn't covered.
