I had been playing on this character for a couple days now, had gone through a handful of gunfights and had spent a lot of time looting around the map. About an hour ago I decided to log off and go watch some TV and take a little break. I come back, start up DayZ again and my character is nowhere to be seen. I click on the server that I had been playing on (US Northeast 0-2) and it says that I have no character alive on the server. I am sure that I didn't die since I logged out myself. I'm a little bummed out that I lost a couple hours of my life that I won't get back now, but the important part is to identify the issue so it doesn't happen to anymore people.
