I clipped it. I was climbing a ladder and my character would be off the ladder, when I reached the top, he flew off the ladder, to his death. I tested it multiple times in different cities, all to the same effect.

The only way to stop it, is to stop climbing and wait for the character to slide back onto the ladder.
