Had inventory opened while I walked backward looking for loot in Novy Sobor police station second story room and walked right through the wall and fell to my death. Just my two cents:
Were you playing on a server to which you had a reasonably high ping? (~ 200 ms)
I've found that if you have a high ping it's very easy to clip through objects and walls.
