Old issue. It could be private shard server. Or just central server lag.

This is a known issue, you most likely have joined a server which was restarted recently and was booting up. Which means the server was not connected to the database yet and therefore your character could not be loaded from the db. For that reason the server has created a new character instead. This will be fixed in future by system which will prevent players from joining servers which are not fully booted yet.
