There is a pool or pond next to the huts west of Khelm that has two sets of stairs leading into the pool and a jetty. I tried to walk half way down the stairs to fill up a water bottle and fell in. If you fall in this pool it is impossible to get out, despite the stairs should allow you to get back out. The character clips through the stairs and the sides are too steep to get out. I had to admit defeat and let the character die
