My Screen will freeze then trap me on my desktop.

Description

Close to every 30-45 minutes my screen will freeze then seconds later put me on my desktop but the thing is that I can still do everything in game but see. When it puts me on my desktop I still receive audio from in game and i can still walk, shoot and do everything that I can do normally but my screen is frozen on my desktop. Even when I alt-tab out and back in it leaves me on my desktop. The only way for me to fix this issue is to access task manager and end dayz. After I relaunch it works fine for 30-45 minutes then it happens again. I have tested this on every most video settings and it still happens. Whether I am in a city or the middle of the wilderness doesn't seem to be a contributing factor.

Additional Information

This first started happening when I upgraded from my old radeon 5450 HD gpu to my new GTX 750ti. I first thought it was a power issue but I have more than enough power to run every thing.
Computer specs
GPU: Geforce GTX 750ti
CPU: Intel quad core 2
Power supply: Corsair 500w
Motherboard: Rev A01
RAM: 4g ddrr2
my computer is a xps 420.
