When you have more that 1 item on quick access bar (1-0), items randomly change to each other, eg. 1-wrench, 3-map. When you are fighting, the game can randomly use other items, in that case opens map

Yeah, Its listed in bugs section in Change Log. It happens when you rapidly click to attack with melee weapon.
