When going up on a staircase my character gets a fractured leg. Usually happens on the Prison buildings. 4x on Prison Building, 1x on a Air Traffic Control bldg, 1x on "School/Office" building.

I second this, happened to me in a couple of the Police Stations I entered, about 5 seconds after reaching the top of the stairs my legs suddenly broke.

I had this is the firehouse, ran to the top and when i reached the top i got 2 fractures fell down and died

This happens to me in most buildings with staircases as steep or steeper than those in apt. buildings. I believe it has an increased chance of happening when you have a momentarily bad connection with the server, and the server/client catching up and resyncing player location may play a role in this.
It's the most common gameplay issue I experience; it's the first bug that's frustrated me enough to feel a need to make an account and report it as well. :P
