There should also be a .mdmp file in the Appdata folder, could you please attach that as well?

Please put the DxDiag, .rpt, .midmp and .mdmp files into a .rar file (using a program such as http://www.rarlab.com/download.htm or similar) and then upload the .rar file.

the .mdmp file you have attached is empty and contains no relevant information which could allow us to help you.
Could you please delete the contents of your "C:\Users\<Name>\AppData\Local\DayZ\" folder, have the game crash again and then upload the newly generated Appdata files in a .rar?
