While in the crouched position and pressing ALT to allow free look you cannot look directly down at the ground.
left,right and up woks fine.
This can make drinking from ponds without getting wet and looting some items difficult.

It is very restricted, but when it blocks your crosshair, with another movement of the mouse it does allow you to aim down but it is very awkward and can cost you valuable time during a firefight, I also noticed you can't even lean from crouch either.
