I have recently had this issue occur twice. Both times in the exact same way.

Whilst holding an AKM, with a 75 round Drum attached with a 30 round magazine in inventory (alongside other attachments on the AKM).

There is a persistent option to reload the AKM, which is easy to accidentally select when performing any action. Once selected a reload animation occurs, the 30 round magazine becomes attached to the AKM and the 75 round magazine disappears.

This occurs regardless of whether both 75 round drum and 30 round magazine are full or partially full. It simply vanishes.
