I have the exact same issue only it happens when exiting the game. I've checked my crash logs and found this:

Reason: Access violation. Illegal read by 0x7ff6d43293a3 at 0x26afa85d5e0

Olga's are spawning in on Ada spawn points and if you approach and interact with the car it has the potential to kill your character. I had one character killed as soon as I got inside to check the fuel level. The car will flip into the air and barrel roll if you remove a wheel from it. I've had this happen on 3 separate occasions.
