Sometimes when one of the quick-swapping buttons is pushed, the character takes out the item, but then immediately puts it back. If this happens it is almost impossible to take out that item and keep it out unless you go into the inventory menu and move the item into the box where items in your hand is.

It seems to be random, but only happens if 1-9 is pressed and there is an item assigned to them.

And sometimes your character doesn't do anything. On more than one occasion I had to press the button multiple times for the animation to start ... and then he shoves the item back into his inventory.
(very irritating when you are getting shot at)
