After finally getting my pistol to stay in my menu from another bug, I unequipped it and checked my inventory to make sure it was still there. I had just climbed up a ladder at some sort of building tower thing (I don't know the map very well) and I went up about a meter and then teleported or moved very fast about a half meter to the right which was over the edge, and my character fell to his death.
