Player switched to "You are dead" screen without warning whilst opening a can of tuna upstairs in north jail at NWAF. Other player in same location saw and heard nothing - the player disappeared leaving no body or items. Dead player respawned on beach and continued game.

I had just now a similar problem, I emptied a ammo box(not the big one, one you can find with some ammo in there) and i instantly died after pushing "empty".
I can remember that I was attacked by invisible zombies just before and I lay down as I emptied it.

This was on a Hardcore server. Normal zombie encounters during the approach to the jail, and the player was standing up when it occurred.
