I've just started playing so it's only happened to me twice so far. When I'm in a building or on a structure with a ladder, I will randomly clip through a wall without actually seeing it happen, then I see the damage log as if I fell, broke my legs, and died. No other players were around. I was fortunate enough to find my body but was unable to loot it, and it was outside of the building rather than inside it.
