I haven't been teleported by ladders in forever. But now i'll get to the top of a ladder, and just all the sudden I'm dead, with a red text message saying my leg hurts. No falling animation, just top of ladder then, DEATH.

Click ladder and let it automatically unequip item in hands
Climb ladder and die at the top.

Happens most when it is raining it seems.

I experienced the same at the fire station in Electro while I was climbing a ladder.
When I was still on the ladder a friend of mine also tried to climb the ladder. In this very moment I died.
Didn't try to reproduce.
