It seems to happen randomly, but occasionally I will try to put something in my inventory (happens with packs, vests, clothes, anything) by dragging it to the empty space, but it won't transfer. No message pops up saying "swap" or whatever, so it's not like there is something in the slot that is invisible. It is literally an empty spot that can't be filled.
