at the very top of the tower blocks access is blocked by rubble (always as) however you could vault over the rubble to gain access, now when you reach the base of the rubble the character does not do the vault animation and therefor you cannot gain access to that floor or the ladder leading to the roof.

Steps To Reproduce
climb to top of the tower block/s walk up to the rubble at the top flight of stairs, press "v"
Additional Information
you can gain access sometimes by drawing a weapon and pressing "w", this will make you run up the rubble.
