Updated report:

->-Using an item in an elevated structure will make you non-visually fall through all floors in the building and take damage appropriate to the height you would fall to the ground.

-Looking back to the last many hours of DayZ, my friend keeps breaking his pants.
He'll be in a building and his screen will flash white as if he took damage and his pants are ruined. He was at a low height from the ground.

-I was at very elevated heights, and died each time.

Original report:

I was sitting still in the top floor of a barn: ( http://img.dayzdb.com/img/buildings/land_stodola_old_open.jpg ), that one.
I was teleported downstairs. A minute later, suddenly, you are dead.

I respawned and ventured off to one of the large industrial buildings.
The one with the crane next to it.
I got to the top floor, wasn't moving anymore.
You are dead.
This happened in a server with a very low population on it, so I don't think I was getting sniped.
As well, I was in third, and didn't see and blood from a headshot, as you do after you get headshot sniped, before getting the you are dead screen.

In both instances I was using an item in my inventory at or just before the time of death.
Situation one: I was emptying ammo out of an ammo box.
Situation two: I was drinking a can of pipsi.

So that may be related to reproduction of the bug.
The bug may be two separate ones.
The barn seemed structural, the latter... I have no idea.
