I was climbing the ladder in the tower on Green Mountain. I opened my inventory about half way up and noticed there was a shirt in the vicinity part (I'd seen this shirt at the bottom). When I tried to turn the shirt into rags I died.
I'm not sure if it was related to that or if it was just random.

Steps To Reproduce
Go to the tower, climb the ladder and try interacting with whatever's at the bottom.
Additional Information
If it was because of the shirt, I suppose it could also be fixed by making it so you can't access the item while climbing (it would be out of reach anyway).
