I have been able to reproduce this one a bit.

i have turbo assigned to left shit along with run.
hands up no weapon,
sprint to a door thats closed, mouse click as i get icon (note do not stop runin forward) as door opens and i collide with moving door, screen will flicker and i will tele to oposite side of house about 3m from closest wall just outside.
