When you are "in pain", (after being hit/shot/taking damage), your character will sometimes start to moan and complain. There is no visual feedback from this (screen shaking), so if it affects accuracy, I am unsure.. But the moaning itself is completely cured by logging off the server and relogging in. Seems painkillers are not necessary due to this.
