It happens when I am in the first story up (or higher) in buildings and my character spontaneously just dies. When I pressed esc to see where I was I had fallen bellow the building. This has happened in a hospital, Air Control tower and a Military Prison. It has happened when moving/still in different stances etc. There are no specific requirements.

At first I assumed it was a hacker but it has happened numerous times.
