If you're in the crouched position at the top of a ladder and attempt to middle click onto it to climb it. Your character will do a step-over animation as if to step over a fence, and in doing so, step off the edge of the building.

Stay in the crouched position at the top of a ladder, move your curser so that you get the arrow pop up that allows you to middle click onto the ladder and press the middle mouse button.
