This usually happens when you have box containers that take 4 slots in the inventory. Certain arrangements of boxes, where there are empty slots between the boxes, will cause one of the boxes to be deleted when you relog. I believe this is because the inventory tries to rearrange items into the topmost available slots, and sometimes this leaves no room for one of more items.

Last night logged out with 2 medkits and a yellow protector case all inside of my taloon backpack along with other items. Logged in today with only 1 medkit, no protector case.
The inventory rearrangement is not at all needed.
