The last time I was sick, I coured my wounds by desinfecting with alcohol, directly after the green message and the sick symbole gone, I used the alcohol again and I was sick again. I think you always get the sick-message even your are not sick, only by using the alcohol for desinfecting.

Yeah it appears that using alcohol tincture will make you sick regardless of whether you've ever been wounded by zombies or not. Luckily it's the 'stage IV' sickness which will cure by itself in about 25 minutes.
