A severe problem I've noticed across almost every server I've been on is that in buildings zombies and players tend to "glitch" out of rooms and run appear from walls. Whether this is simply a matter of desycn or an actual glitch I'm not too sure as on some occassions people have actual ran out of a building with the use of a door.
