Sometimes if I'm prone and looking around with my mouse while not hitting any keys and begin moving forward (Pressing W) my characters legs will move as if I'm moving but I will not be, and then the game will just roll me. It's almost as if the game detects I'm stuck and tries to self-correct.

It may only be in certain areas (around certain objects) but I know I've had to happen on top of roofs as well as on the ground.
