Quick access to items via Quickbar and correspondig hotkeys (1, 2...) requires hitting the hotkey for several times before the character takes the item into hands.

e.g.: axe in hands, i want to switch to M4 (hotkey "2"): hit "2", nothing happens. hit "2", nothing. 3rd time hitting 2 -> switch to m4.
