I was punching a zed while we both bled out but the zombie knocked me out unconcious and I wanted to respawn, the game displayed the usual messages "scene loaded; testing 1 2 3" but nothing after that happened, all I got was a "Please Wait" message.

Maybe if you were punching a zed and you got yourself unconcious it would happen again, but I can't really generalize.
