I had a full inventory and reloaded my M4 with a 10rd CMag from a 60rd mag and the mag technically had "nowhere to go" so it disappeared. I could not find it on the floor, I waited to see if it would appear and didn't and can only assume it was confined to oblivion. Reproduced with reloading from 30rd PMag to 10rd CMag and a standard stanag mag to 10rd CMag.

Must have a dual inventory slot magazine in your rifle (ie. PMag, Stanag Mag, dual stack Stanag) and must have a single slot magazine (ie. 10rd CMag) anywhere in your inventory. This can be reproduced in either of two ways, fill your entire bag with stuff or set up your bag so even when the CMag is used, there are not two slots in conjunction with eachother to allow the dual slot mag to exist in the bag. Once that is done, press "Use" with your rifle out to reload to the next mag (CMag) and it will reload to the CMag and cause the previous mag to disappear.
Could happen with other item combinations where you can switch from a dual slot item loaded onto the item (ie. portable cooker/gas lamp and gas cylinders) to a single slot with no space remaining.
