I was climbing up the ladder in the tower at Green Mountain after engaging two bandits, I had not been shot, my status was perfectly healthy, mid-way the ladder climb my screen turned black and said "You are dead", for no reason what-so-ever.
I was with friendlies so they had my back covered so no other players could shoot.

Unfortunately, the problem regarding climbing the ladders is a known issue that has previously been reported.
