If you descend the ladder from the roof to the ground you get stuck behind a rubbish pile which can not be crossed. Even if you use the step over function to clear it, as soon as you press a movement button you are pulled back to the bottom of the ladder.

To get out, you have to climb back up the ladder and exit the hospital the normal way.
