- Die. 2. Exit the game. 3. Go to "Change Servers." 4. Select the same server you were in (this is the way I did it everytime, so it may not be dependent on the server, but that's how it worked for me).
CONFIRMED: This does not work when you go unconscious.

This does work when you go unconscious, just have to have a friend shoot you. Leave server, go into a new server you'll be back where you were. leave that server and go back to the server with your friend. You are now no longer unconscious. Both my friend and I were able to duplicate this behavior.

I had this problem. I got killed at the military base between Vybor and Stary. The game acknowledged I died. I came back on the game later this afternoon (about 30 minutes after the death occurred) on a different server and I was still alive.
I don't believe I clicked 'respawn' once I had been killed.

Has happened a few times to me.
- I was in fire station and guy shot me. I rejoin server, it resurrects my character in the same spot. The guy who shot me was quite surprised!
- My friend got shot by someone at NWAF. He rejoins and resurrects character with all gear.
- I go unconscious so my friend shoots me dead, I rejoin and resurrected and conscious again. ie also only cure for unconsciousness at the moment!
