Had a pipe wrench in slot "1". Found a fire ax and attempted to place it in slot "1" also, assuming that it would drop the pipe wrench automatically. The ax did not appear and the pipe wrench texture turned white instead of red as it had been and was no longer functional. Dragging and dropping the item on the ground made it disappear and neither item was accessible any longer.
