If you respawn on/near the 'office' at Air Station Mike on Stratis, you can spawn half-way under the floor. You are then stuck, with no way out. You can't kill yourself (explosions won't hurt you, nor will gunshots) and if you press respawn, you just respawn in the same place.

Steps To Reproduce
I'm not entirely sure if this will work, but I'd try:
Go to the 'office' building at Air Station Mike on a game mode that respawns you where you died (it happened to me while playing Escape from Stratis).
Die inside the office.
You respawn half-way 'below' it and are then forever stuck.
