Bohemia Official Campaign Triggers Broken

I repeatedly have broken triggers on East Wind. Every single time I play Blackfoot Down, the charge I place is not counted as being placed. I literally deleted my saves, reinstalled the game, and ran the game without mods, and every single time, the charge fails to count. In addition, on Crossing Paths, Kerry goes MIA within a minute of picking up the rucksack with the spare NVGs and following the squad to Nikos. Sometimes I'll make it to Nikos, other times I won't even make it there before the mission fails. And finally, on Contact's Second Mission, Going Dark, I can get into the truck, and before I make it to the sharp 90 Degree right turn with a Roadblock, I mission fail for friendly fire. This has been happening for the last several months and has only been getting worse.

Steps To Reproduce

BLACKFOOT DOWN
1: Start East Wind Campaign without Mods and play Through to Blackfoot Down
2: Get the Mission Objective to place the Explosive Charge.
3: Place Explosive Charge on Blackfoot Wreckage after Objective is Given.

Observed: Mission Fails to Progress due to explosive charge trigger failing to fire. The only way to progress is to use the "End Mission" Cheat.
Expected: Explosive Charge is counted as being placed on Blackfoot and Mission Progresses.

CROSSING PATHS
1: Continue playing East Wind without Mods to Crossing Paths
2: Regroup with Fireteam and pick up Carryall Pack.
3: Move with Squad to Nikos and Mission Fail due to MIA.

Observed: Mission Fail State where you're classed as MIA. The only way to progress is to use the "End Mission" Cheat.

Expected: Mission continues as normal allowing you to group with Nikos and progress the Mission.

CONTACT GOING DARK
1: Spawn in and do the steps prior to boarding the HEMTT
2: Board HEMTT and start driving
3: Mission Fails due to Friendly Fire when you didn't hit a single friendly

Observed: Empty Roadblock on the way to the burn pit that normally has Friendlies at it, mission failing shortly before or after arriving at it due to friendly fire
Expected: Friendlies at Roadblock, No Mission Failure for Friendly Fire, Able to proceed as normal with the Mission.

Additional Information

There are very likely more broken mission triggers besides those three. I gave up trying to debug stuff after Crossing Paths.
