If you go into the inventory and add / remove items from the quick slot using a controller you get an odd closeup of the torch appear in the screen that you can't get rid of. Seems to happen in all game modes and on xbox and pc.

I've done a little more testing - the glitch seems to happen when you try and remove an item from quick bar.
