Steps To Reproduce
I picked up a shotgun and loaded it with slugs and instantly died. Was in fairly good condition, no reason for me to die that I am aware of.

Adding to this. It could improve the game to add a simple "You died from X" instead of just "You are dead". Anyway I do think its some sort of glitch. Thanks.
